OCULAR INFLAMMATION

Inflammation can affect various structures in the eye –the lids and lashes, the conjunctiva and cornea, the sclera and the internal structures of the eye. Such inflammation is most often due to underlying systemic diseases or a dysregulation of the immune system of the body.

Inflammation of the lids and lashes are termed meibomitis and blepharitis and often coexist with seborrheic dermatitis (dandruff) and acne (pimples). Although the condition often results in symptoms like recurrent redness, irritation, burning sensation and sicharge, it can sometimes result in significant corneal damage – including the growth of blood vessels, and sometimes hole formation in the corneal tissues.

Some inflammations such as Mooren’s ulcer often have no discernable cause, but the ulceration in the cornea can cause significant distress to the patient and often affects visual function. After thorough investigations, the treatment requires the use of oral steroids and immunosuppressive medications, in addition to eye drops. In cases with advanced corneal thinning, in addition to these measures, surgical management may be required.

The white coats of the eye can also be the seat of inflammatory changes – resulting in redness, pain and swelling on the ocular surface. Such conditions are termed episcleritis and scleritis. While medical management will often result in resolution of these conditions, in advanced cases, surgical measures may be required.

Inflammation within the eyes is termed uveitis, and often has a systemic cause. Appropriate investigations and treatment can help reduce the inflammation in such eyes, helping to preserve vision. If not treated at an early stage, such conditions can result in significant ocular damage and morbidity.
